Required Documents for All Applicants

• Printed copy of Sponsor’s Address and Phone Number in the UK
• If applicant is currently PREGNANT, please secure a clearance to undergo a Chest X-ray from your Obstetrician
• If with previous history of Tuberculosis and treatment, please bring medical records and previous X-rays images for comparison

Additional Required Documents for 18 Years Old and Above

• Passport (original and photocopy)
• 2 Original government issued IDs (original and photocopy) (One ID to be presented at lobby building security and one ID to be presented upon registration.)

Additional Required Documents for 17 Years Old and Below

• Passport (original and photocopy)
• Current School ID (original and photocopy)
• PSA Birth Certificate

• Signed authorization letter from the child’s parent + Copy of parent’s passport

  1. These can be emailed to Guardian and Guardian can print this. Authorization letter and copy of passport must be on hand on the day of the medical.
  2. Guardian’s complete name and ID should be part of the authorization letter
  3. Government Issued ID of the Guardian
